Statement of Intent
The information below outlines how our practice intends to meet requirements set out by the NHS contract relating to Patient Online Services from April 2014. Some of the requirements may be subject to the necessary NHS GP systems and software being available to the practice but are still in line with the current 15/16 contract.
All practices must include the NHS Number as the primary identifier in all NHS clinical correspondence issued. The use of this verified NHS number ensures that patients can be correctly identified and supports safe, high-quality care.
Morden Hall Medical Centre can confirm that we use the NHS number as an identifier for all patients.
Electronic Appointment Booking
Practices are required to promote and offer the facility for all patients to book and cancel appointments at the practice online.
Morden Hall Medical Centre can confirm that we currently offer the facility of booking and cancelling appointments online to all patients. Please sign up for this service at reception.
Online Booking Of Repeat Prescriptions
Practices are required to promote and offer the facility for all patients, who wish to, to order online, view and print a list of their repeat prescriptions for necessary drugs, medicines or appliances.
Morden Hall Medical Centre can confirm that we currently offer the facility of ordering repeat prescriptions online for patients over the age of 18. Please sign up for this service at reception.
Summary Care Record
Having your Summary Care Record available will help anyone treating you who does not have available your full medical record. They will have access to information about any medication you may be taking and any drugs that you have a recorded allergy or sensitivity to. Morden Hall Medical Centre can confirm that we already currently offer the following facility for patients:
- To book, view, amend, cancel and print appointments online
- To order online, view and print a list of their repeat prescriptions for drugs, medicines or appliances
- To view online, export or print summary information from their record, relating to medications, allergies, adverse reactions and any other items agreed between the practice and individual patient
This practice also plans to offer the facility for patients to view online, export or print detailed coded information held in their own records by 31st March.
GP2GP record transfers
If you move to a GP surgery that uses electronic medical records then your patient records will be available to them shortly after registering you at the practice. This is called GP2GP. It is very important that you are registered with a doctor at all times. If you leave your GP and register with a new GP, your medical records will be removed from your previous doctor and forwarded on to your new GP via NHS England.
Morden Hall Medical Centre can confirm that GP2GP transfers are already active, and we send and receive patient records via this system.
Patient access to their GP record
Practices are required to promote and offer the facility for patients to view online, export or print any summary of information from their records relating to medications, allergies, adverse reactions and any other items/date such as ‘additional’ record elements which has been agreed between the contractor and the patient.
Morden Hall Medical Centre can confirm that patients can request access to their medical records via Patient Access. If you wish to have access to this information and are not currently registered with Patient Access then please contact a receptionist to register.
