Before filling out a registration form, check you are in our catchment area.
Important: If you live outside our catchment area
If you proceed with this registration while living outside our catchment area, you will be classified as an out of area patient. This may result in delays if you are referred to secondary services, and you will not be eligible for home visits. If you require further information before registering, please contact the surgery.

Registering with the surgery
To register with the surgery:
- fill out a new patient registration form online
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
